Pediatric Phimosis: Causes & Treatment

Dr. Muhammad Faridul Islam
Author
Phimosis is a common condition in children where the
foreskin (the skin covering the penis) cannot be pulled back to expose the head
of the penis (glans). It is often noticed at birth or in early childhood.
Causes of Phimosis
- Normal
development: In many children, the foreskin covers the glans at birth
and gradually retracts as they grow. In some cases, this process is
delayed or does not happen naturally.
- Infections:
Repeated infections under the foreskin can make the skin tight, leading to
phimosis.
- Injury:
Trauma or swelling of the foreskin may also cause the condition.
Symptoms of Phimosis in Children
- The
glans cannot be fully seen.
- Difficulty
or pain while urinating.
- Urine
dribbling or spraying.
- Pain,
swelling, or irritation of the glans.
- Pus
buildup under the foreskin.
Treatment Options
- Observation:
In many cases, especially in children under 3–5 years old, phimosis
improves naturally over time. Doctors may recommend waiting and
monitoring.
- Steroid
cream: Sometimes doctors prescribe topical creams to soften the
foreskin, making retraction easier.
- Surgery
(Circumcision): If conservative treatment fails or complications
arise, surgery may be required to remove the foreskin.
Benefits of Surgery
- Complete
removal of foreskin.
- Easier
urination and improved comfort.
- Lower
risk of infections.
- Better
genital hygiene.
Risks of Surgery
- Bleeding
- Infection
- Reaction
to anesthesia
When to See a Doctor
Seek medical advice if your child:
- Cannot
retract the foreskin at all.
- Experiences
pain or burning during urination.
- Shows
pus buildup under the foreskin.
- Faces
repeated urinary problems.
